9) List additional information that would be helpful in determining what kind of dog would best suit your lifestyle.

10) Is this your first Leo? Please describe the animals that currently reside in your home.

11) Please describe your family members who will be residing with the dog.

12) Do you live in an urban, suburban, or rural area? Where will the dog stay during the day and night?

13) Is your yard fenced or dog friendly? If not, would you be open to fencing your yard? How large is your yard?
